• Caring Connections

  A program of the National Hospice and Palliative Care Organization (NHPCO) that provides free resources on end of life care issues and services, including information on hospice providers and state-by-state advance directives.

• The Dartmouth Atlas of Health Care

  The Dartmouth Atlas of Health Care is an interactive website that uses Medicare data to provide information and analysis on national, regional, and local heathcare markets, including individual hospitals and their affiliated physicians.

• Medicare.gov

  Medicare.gov is the official U.S. Government site for people with Medicare.

• The National PACE Association (NPA)

  The NPA is an organization that works to advance Programs of All-inclusive Care for the Elderly (PACE), which coordinate and provide preventative, primary, acute and long term services so that individuals eligible for nursing home care can continue living in their community.

  A Medicare health plan designed to treat nursing home patients in the nursing home.

• The American Association of Retired Persons (AARP)

  The AARP is a non-profit, nonpartisan advocacy group for people age 50 and older.

  An initiative to encourage patients to express their wishes regarding healthcare and for providers to respect those wishes.
